New Member Sponsorship Form

Submission deadline: 4 p.m. on Friday, April 18, 2025

Please download a New Member Sponsorship Form and meet with your Prospective New Member(s) to complete the form together. Sponsors must submit the form and a photo of their Prospective New Member(s) by 4 p.m. on Friday, April 18, 2025. Forms may be submitted in person to the Jewel Charity office or via email to avery.henley@jewelcharity.org.

Interviews

Sponsors must attend an in-person interview with the Membership Committee on one of the following dates. Please click below to sign up for an interview time.
